In order to be able to offer you the services of dental care and treatment, we collect the following information from you:

• Full name

• Address

• Telephone number

• E-mail

• Sex

• Date of birth

• No. of identity or passport

• Profession

• Medical and dental history

• Data of the referring doctor

• X-rays

• Results of a clinical laboratory

• Any other information that may be related to care and treatment

• Details regarding health insurance

Purpose

The observance and processing of this information, which will be updated at regular intervals, is a legal obligation under the General Data Protection Regulation (EU) 2016/679, as it was adopted by the Republic of Cyprus and is now part of the national Law (125 (1) of 2018) on the Protection of Individuals with regard to the Processing of Personal Data for the Free Movement of Data and any subsequent amendments thereto. It is necessary for the verification of identity, the need for communication, as well as for reminders of periodic checks, the provision of dental services and / or treatments offered by us and the monitoring of your health and compliance with legal obligations and / or for accounting purposes.

These data are retained for as long as the purpose for which they were acquired. We may retain your data for a longer period of time in cases where we cannot delete them (for legal, regulatory or technical reasons) but in any case not beyond fifteen (15) years after the termination of our contractual relationship.

Rights

You have the right to access your data and you can receive a copy to verify that we process the data legally. You can ask for your details to be corrected and / or deleted. Your request for deletion may not be satisfied if there are legal reasons that we will share with you. You may also ask for withdrawal of your consent (considering that we may not be able to continue offering you dental services / treatments ).

You have the right to request a limitation on the processing of your personal data if you find that it is inaccurate or has been used illegally or is no longer needed and to oppose cases where we process your personal data for marketing purposes.

Transmission to third parties

Your data are held in a secured electronic and / or written form and are confidential. They will not be disclosed to third parties unless it is necessary to offer the right dental treatment and the services for which you have appointed us or if we are legally bound or if you have authorized us to do so. We may need to disclose your personal information to the following persons without your specific prior notice being required:

• A partner doctor or dentist or dental technician.

• Radiological centers, histopathological centers, clinical laboratories or other diagnostic laboratories.

• Insurance companies covering your health care.

• Accountants, auditors or other professional consultants.
